About the service
The Single Point of Access is staffed by experienced clinicians from both primary and secondary mental health services (adult and older adult) to ensure that all referrals are assessed and allocated to the most appropriate service.
All routine referrals should be sent electronically. All routine referrals will be received via email to the Single Point of Access on the following email address: WHCNHS.amhreferrals@nhs.net .
The Single Point of Access is in place to assess quickly, accurately and efficiently based on clinical information given in referrals and historical information that can be accessed at time of triage to allocate the best service to meet the patient’s needs.
The Single Point of Access aims to assess routine referrals within a 48 working hours time period.
Urgent referrals should be telephone through to the Single Point of Access on 01905 681477.
If you need a copy of the referral form, you can contact the administration team on the Single Point of Access telephone number and request a copy.
Health Care professionals with access to Carenotes (our electronic patient records) can complete the referral form on Carenotes (under referral tab). Complete an email and send to the generic email informing Single Point of Access of the referral containing the NHS number of the patient.
Single Point of Access clinical staff also offers advice on mental health services to health care professionals-this can be accessed by contacting the Single Point of Access via telephone.
