Parkinson's Disease Specialist Nurses

Services are delivered through combination of phone support as well as face to face appointments either within the client’s home or at an outpatient clinic.

Services we provide

The aim of the service is to enable people with Parkinson’s disease to live with their limitations and to empower them to manage their condition to live the best quality of life possible.

Services are delivered through combination of phone support as well as face to face appointments either within the client’s home or at an outpatient clinic.

Who is the service for?

All adults living with Parkinson’s Disease who have an established diagnosis of Parkinson’s disease.

What does the service offer?

Parkinson's disease is a condition that affects the brain and nervous system. It is a progressive condition that causes problems like shaking, slowness, and stiffness that gets worse over time.

Every person affected by Parkinson’s will have a different journey and our Specialist Nursing Team are there to offer advice, support and care planning which is individual for each person.

Clinical Nurse Specialists can make a positive difference with their knowledge of symptom relief, assisting in making choices, identifying other supporting services and linking in with a Multi-Disciplinary Team. This can restore confidence and control back into the lives of patients and their families/carers.

  • A specialist service with people in the community setting, which strives to maximize functional independence by promoting self-management, with the overall aim to enhance the individuals’ quality of life
  • The service adopts a ‘person centred’ approach, co-ordinates care and works collaboratively with other agencies as required
  • Advisory service to other clinicians.

Make a referral

Who can make a referral?

Any Health Care professional who is able to provide the appropriate clinical information regarding the client, their diagnosis and needs but the patient must have a confirmed diagnosis of Idiopathic Parkinsons Disease.

Please ensure enough detail is provided to ensure the referral can be triaged by the team and actioned promptly

Acceptance Criteria

  • Over 16 years of age
  • Confirmed diagnosis of Parkinson’s Disease which has previously been diagnosed by a Neurologist or Specialist Parkinson’s Elderly Care Physician.
  • Registered with a Worcestershire GP

Urgent Criteria

  • Newly diagnosed with Parkinson’s Disease

Routine Criteria

  • Service users with any concerns re drug side effects.
  • Falls related to postural symptoms/low BP.
  • Living alone
  • Sudden onset cognitive changes where acute cause has been ruled out
  • Acute/Community wards asking for our advice.
  • GP’s who have concerns and are asking our advice.
