Older Adults Community Mental Health Team - Worcestershire

- Service hours: Monday to Friday, 9:00am to 5:00, excluding Bank Holidays. Out of hours, the 24-hour Mental Health Helpline is available if urgent support is required.
About the service
Mental health needs
The service provides multi-disciplinary assessments, care planning, interventions, and treatment for older adults with complex and/or enduring mental health needs (not dementia). The services promote recovery and independence for patients and offer education and support for carers. The service is accessed through the Mental Health Single Point of Access, or from Mental Health Inpatient Wards. The service is for people aged 65+ or people with significant frailty under 65 when an Older Adult service would be more appropriate.
Dementia and Cognitive Care
The service provides multi-disciplinary assessments, care planning, interventions and treatment for older adults with dementia. The services promote recovery and independence as far as possible for patients and offer education and support for carers. The service is accessed through the Mental Health Single Point of Access, or from Mental Health Inpatient Wards. The service is for people of any age with Dementia. This service also provides a comprehensive assessment and diagnostic service for people with suspected dementia who have limited insight into their needs and require support with their activities, if daily living.

Make a referral
Health and social care professionals can refer direct to the teams by email (see above under team locality information).
All clinicians external to the Trust must make referrals to adult mental health through our Single Point of Access: 01905 681477 or Single Point of Access Email Address
You can be referred to the service by any health and social care professional such as your GP or Community Mental Health Team.
An urgent referral for mental health support out of hours should be made via the 24-Hour Mental Health Helpline.
