“Working together with patients, families, colleagues and the wider community for outstanding care which provides specialist mental health support to enable older people to live well and thrive.” The Herefordshire Older Adult Community Mental Health Team (OACMHT) provides a service for people with mental health difficulties who are over the age of 65 and registered with a Herefordshire GP.

About our service
The Herefordshire Older Adult Community Mental Health Team (OACMHT) provides a service for people with mental health difficulties who are over the age of 65 and registered with a Herefordshire GP.
We offer assessments of your mental health or dementia difficulties to inform the type of care needed. This may lead to time-limited support and treatment. Support and treatment may include signposting, medication, psychological therapy, occupational support, a variety of groups, and safety planning.
We are part of the mental health and dementia services pathway in Herefordshire. We work with GPs and other mental health and dementia services in Herefordshire to ensure that you receive the right care at the right time.
What we do
In our team, different professions may be able to support you with your difficulties including, Mental Health Nurses (CPN), Support Workers, Psychologists, Occupational Therapists (OT), and Psychiatrists.
Mental Health Service:
The service is open for people with common and complex mental health disorders. For example, anxiety, depression, psychosis, PTSD, and trauma. We offer this service to people with more complicated presentations with increased risk.
Dementia Service:
We provide a service for those who have a diagnosis of dementia and whose needs are more complicated. This service is provided to people of any age.
What we offer:

What to Expect
We offer person-centred care designed around your strengths and needs. We also ensure you receive:
- A confidential service.
- A prompt assessment appointment usually within 28 days.
- Clear and timely communication.
- Compassionate and respectful approach.
- Personalised planning.
In return, we ask:
- That you are involved in planning your care throughout your time with the service.
- That you take an active role in your personal recovery and be willing to share your thoughts and feelings.
- That you are prepared to try different treatments and be inclined to give things a go.
- That you attend your appointments or provide cancellation with 48-hours’ notice.
- For respect and consideration towards staff.

Duty Desk:
As part of the support the team provides, there is a telephone support line to a member of the team that deals with urgent queries/concerns related to patient’s care. Please ring if your question is urgent and you cannot contact your normal team member, via our main telephone number: 01432 220445 (Monday – Friday 9am – 5pm).
