Local partnership wins ‘NHS Team of the Year’ at prestigious awards

Health and social care staff from across Worcestershire have been named ‘NHS Team of the Year’ at the Worcestershire Health and Social Care Awards 2024.

The winners, jointly known as the Care Transfer Hub, are a partnership between Health, Housing, and Social Care organisations in Worcestershire. The partnership includes:

·       Onward Care team – Herefordshire and Worcestershire Health and Care NHS Trust & Worcestershire County Council

·       Community Single Point of Access - Herefordshire and Worcestershire Health and Care NHS Trust

·       Capacity Management team - Herefordshire and Worcestershire Health and Care NHS Trust

·       Reablement triage – Herefordshire and Worcestershire Health and Care NHS Trust & Worcestershire County Council

·       Homelessness Liaison – Worcester City Council

·       Continuing Healthcare – Herefordshire and Worcestershire Integrated Care Board

Hosted by Newsquest, the awards ceremony took place on 19th September at the Bank House Hotel in Bransford, with organisations across health and social care in Worcestershire in attendance.

James Womersley, Head of Patient Flow and Care Navigation at Herefordshire and Worcestershire Health and Care NHS Trust was delighted with this recognition, “It is great that the Care Transfer Hub has been acknowledged for the fantastic work it does every single day. Huge thanks to all the staff involved in this partnership – they work so hard to support patients to access the right care in the right place at the right time.”

The Care Transfer Hub plays a vital role in supporting patients with complex health, care, and housing needs, ensuring they can access the necessary services within the community.

This improved coordination between teams has led to a more streamlined and integrated approach across the health and social care system, resulting in better outcomes for patients.
