Cromwell House



Eating Disorder Service (Worcestershire)
Assessment for people who suffer with eating disorders. We work collaboratively with patients to work towards recovery and offer a range of interventions that are tailored to individual need. We provide support and information for families, carers and other health professionals.



Gateway Workers
Provides assessment and consultation for adults experiencing common mental health problems (anxiety and depression).

Harvington Ward


 
Holt Ward

An 18 bedded Mental Health in-patient unit for males aged between 18-65 years in which risk dictates that they require assessment/treatment in hospital either in a voluntary or involuntary basis.

 
Home Treatment Team (Wyre Forest)

Service for patients in Wyre Forest 16 and over to be treated in their own homes as an alternative to hospital admission to psychiatric ward. We also facilitate early discharge for patients admitted.

Keith Winter Close



Mental Health Liaison Team based at the Alexandra Hospital
Assessment of patients who have presented with deliberate self harm / attempted suicide or mental health issues through Redditch A&E. Assessment of patients on medical wards following DSH/ attempted suicide.


 
Psychiatric Intensive Care Unit (PICU) (Worcester)
Offers multi-disciplinary assessment and treatment of individuals whose level of risk make it inappropriate for them to be placed an a general adult ward.



Reablement Service Hub (Worcestershire)
Increase community based valued roles and activities, including reducing social isolation by providing people with opportunities to extend their social networks and form relationships not only with other people with mental health difficulties and staff, but also with people outside the mental health system.

Recovery Inpatient Service

Inpatient assessment and treatment for people with severe and enduring mental illness.

Rehabilitation

The service concentrates on maximising physical, emotional and spiritual well being, encouraging individuals to identify recovery goals. We support the individual in their recovery process and work in partnership at their pace.

Shrub Hill Workshop
Shrub Hill Workshop provides a service for people in mental health recovery moving towards college, employment, volunteering or other community based activities.


 


Tudor Lodge
Registered care home providing social care to people with enduring mental health needs, excluding persons with a learning disability or dementia and persons who are subject to the mental health act.



Vocational Services
Assistance towards employment, education or volunteering: Specific services a) Individual Placement and Support to employment b) Trust vocational centres (Link Nurseries, Orchard Place Workshop, Shrub Hill), c) Job Retention, d) Exemplar Employer Scheme.


 

Worcestershire's Early Intervention Service
Worcestershire's Early Intervention Service addresses the needs of young people (aged 14-35) with a first episode of psychosis or of bipolar disorder (specifically bipolar l).  We also offer a time-limited intervention to young people with an At Risk Mental State (ARMS). 



Worcester Royal Hospital Mental Health Liaison Team
Responding to all mental health referrals from A&E, and to self-harm and suicide assessment requests across the hospital.

Children, Young People and Families:



Audiology

A county wide service providing the coordination of the Newborn Hearing Screening Programme,  a hearing assessment in reception year and targeted assessment of pre school and school aged children in the community.



Child and Adolescent Mental Health Services
CAMHS is a multi-agency team of psychiatrists, psychologists, psychotherapists, community psychiatric nurses, primary mental health workers and therapeutic social workers to provide services county-wide.  The service specialises in delivering assessment, support, therapeutic intervention and treatment for children and young people with complex and enduring emotional, behavioural difficulties and emerging mental health disorders. The service includes access to 24 hour CAMHS expertise, advice and support within acute hospitals settings, expertise within the integrated service for looked after children, Youth Offending and Early intervention for psychosis. CAMHS also works with children in the community requiring very specialist in-patient treatment, often preventing admission.



Child Development
Child Development Centres provide a holistic assessment of pre school aged children with physical disability on an individual basis and whilst part of a group.



Community Paediatrics

A county wide secondary care service providing specialist health assessment and therapeutic input for children with learning and physical disability. Assessment and management of children with neurodisability including autism and ADHD. Assessment and support for children within the looked after system as well as providing advise to the adoption and fostering panels. Statutory advise to the education department when assessing children needs within school. Medical assessment of children with urinary and bowel incontinence. Medical assessment for children with hearing loss.


 

Health Visiting
A county wide service providing universal services as part of the Healthy Child Programme to all children from birth to starting school.  This includes new birth visits, breast feeding support, newborn hearing screening, development checks and targeted support for vulnerable families.


 

Ludlow Road
Six-bedded short break unit in Kidderminster providing short breaks for children and young people with learning disability, physical disability and complex health needs.

 

Orchard Service
Community based nursing service to prevent admission, aid discharge and for end of life care. The service includes the Support for Families Team (a clinical psychologist, community play specialist and social worker) who provide support for families of children with complex health conditions and the Home Support Team who care for children in their own homes overnight.


 

Special School Nursing and Child Development Centres
Child Development Centres provide a holistic assessment of pre school aged children with physical disability on an individual basis and whilst part of a group. Special school nurses are usually based in special schools and provide a nursing service to children in those schools together with the provision of training for school staff.



Paediatric Occupational Therapy
A countywide service for the assessment and treatment of children and young people who require occupational therapy to achieve their goals.  The service includes play therapists in the Wyre Forest area.



Paediatric Physiotherapy
A countywide service for the assessment and treatment of children and young people who require physiotherapy to achieve their goals.


 

Paediatric Speech and Language Therapy
A service for children and young people with speech language and communication needs. Provided for pre school children from Children's Centres and in school for school aged children and young people. Training provided for schools and external agencies.



Safeguarding Adults and Children



School Health Nursing
 

A county wide School Health Nursing service which focusses on health prevention. Specific services are drop ins to all schools in the county, immunisations, health reviews on school aged looked after children and participation in the provision of services for children subject to child protection plans and child in need plans.



SPACE

Inpatient, outpatient and community care on a locality basis to adults and older adults across Worcestershire. With five community hospitals, four older adult mental health inpatient wards, district nursing, therapists and podiatry, as well as numerous specialist services such as tissue viability, this is the Trust’s largest service delivery unit.

Adult Speech & Language Therapy 

Countywide assessment, case management, treatment , carer support, education & training services in the Acute and Community Hospitals, Community Outpatient clinics, Domiciliary, Specialist teams, Rehab and Stroke Units, etc.


   

Admiral Nurses
Information, advice and emotional support for families. Supportive educational and consultancy role to professionals, voluntary sector and communities supporting people with dementia.



Chronic Fatigue

They can make a positive difference with their knowledge of the condition and ability to explain and advise on the management of difficult
symptoms. The can also identify other supporting services and link in with a multidisciplinary team.

Early Intervention Dementia Service

This service is available to people of any age, registered with a GP in Worcestershire who are experiencing memory problems and do not have a formal diagnosis of dementia.


 
Enhanced Care Team

The team offer rapid response intervention to prevent hospital admissions and facilitate early discharges from inpatient settings.

The team is made up of Nurses, Physiotherapists, Occupational Therapists and Healthcare support workers. Also included are the Evening and Night District Nursing team.


  
Expert Patient
The EPP Self Management Course is a free course for any adult, who has a long-term health condition(s) and who would like to find ways of managing their illness more positively in order to improve their quality of life.



Health Trainers
Provide advice and support to help people live healthier lives.

Lymphodema
The Lymphoedema Team offer specialist support, advice and management of complex symptoms for patients with primary, secondary or mixed oedemas.



Minor Injuries Units (MIUs)
Patients with a wide range of minor injuries including cuts, grazes, wounds, sprains, strains, minor burns and broken bones can be treated at MIUs at Bromsgrove, Evesham, Kidderminster, Malvern or Tenbury.



Multiple Sclerosis (MS) Specialist Nurses
They can make a positive difference with their knowledge of symptom relief, assisting in making choices, identifying other supporting services and linking in with a multi-disciplinary team. 
 
Neurology
Worcestershire Integrated Neurological Rehabilitation Service (WINRS) is a specialist service with people in the community setting, which strives to maximize functional independence by promoting self management, with the overall aim to enhance the individuals’ quality of life.



Neuropsychology

Neuropsychology tries to understand how the brain affects what we do and how injury to the brain changes what we do.



Nurse Advisors to the Elderly
Nurse Advisors for older people promote the health and independence of older people living at home.



Occupational Therapy
Work with you and your family or carers to identify any areas of difficulty with daily living tasks - including washing, dressing, cooking and getting on and off your bed, toilet and chair.



Older Adult In-Reach To Care Homes Team
Referals can only be made via the Community Mental Health Teams and the service currently only covers the south of the county.


 

Older Adults Mental Health Inpatient Units (Athelon Ward and Newhaven)
The wards support service users over the age of 65 who are suffering with mental health needs (functional illness) which requires in-patient treatment.



Parkinson's Disease Specialist Nurses
They can make a positive difference with their knowledge of symptom relief, assisting in making choices, identifying other supporting services and linking in with a multi-disciplinary team.

Physiotherapy Domiciliary

This service treats people in their own home. Patients may have a musculo-skeletal or neurological/ long term condition or may be post surgical and are housebound by their condition. The priority is to work as part of a MDT to rehabilitate and gain optimum improvement in function, to keep patient independent and safe at home.



Physiotherapy Element of Falls Prevention

This service provides a holistic assessment to identify pre-disposing factors to falling and a management plan to address these and reduce the risk of further falls.



Physiotherapy Musculo-Skeletal 
This service treats people with musculo-skeletal conditions including Low Back Pain, neck pain, osteoarthritis sports injuries and post surgical conditions. The priority is to rehabilitate patients back to full function.



Podiatry Service
This service aims to provide high quality accessible foot health services for the people of Worcestershire.

Tissue Viability
Tissue Viability is an area of healthcare that has synergy with many medical specialties - especially general surgery, vascular surgery, dermatology, care of older people, paediatrics, orthopaedics, and diabetology.



Virtual Ward (Wyre Forest)
The Virtual Ward aims to reduce hospital admissions by identifying patients who are at high risk of admission and managing them more effectively in the community.



X-Ray 

Worcestershire Acute Trust Service.

Adult and children’s respite, outpatient and community contact activity for people with Learning Disabilities and their families, mainly for the population of Worcestershire. Services are delivered by locality between the North and South of the county. There are also countywide professional leads for nurses, occupational therapists, physiotherapists, social workers and speech and language therapists. The service operates with a Section 75 agreement, supporting the service to offer an integrated seamless pathway, for people with a learning disability.

Church View Short Breaks
A specialist short breaks service providing short breaks to adults with various degrees of behaviours which challenge and complex health care needs.



Community Learning Disability Team (Bromsgrove/Redditch)

We are an integrated health and social care team providing a comprehensive community service to adults with a learning disability living within the Bromsgrove and Redditch areas of Worcestershire.



Community Learning Disability Team (Malvern/Wychavon)

We are an integrated health and social care team providing a comprehensive community service to adults with a learning disability living within the Malvern and Wychavon areas of Worcestershire.



Community Learning Disability Team (Worcester/Droitwich)
We are an integrated health and social care team providing a comprehensive community service to adults with a learning disability living within Worcester and Droitwich.



Community Learning Disability Team (Wyre Forest)
We are an integrated health and social care team providing a comprehensive community service to adults with a learning disability living within the Wyre Forest area of Worcestershire.


 

 

 
Epilepsy/EEG Team
The EEG (Electroencephalography) service offers departmental recordings of routine, sleep and overnight ambulatory EEG to adults and children. A mobile service is offered to visit clients with LD and challenging behaviours at home or in school or day centres if a visit to hospital is too stressful. A copy of the EEG referral letter can be obtained from the department secretary for use by the GP or Epilepsy nurse.



Learning Disability Psychiatry
Assessment and treatment of psychiatric, behavioural and seizure disorders associated with learning disability in adults. Service to children with severe learning disability and complex behavioural or psychiatric problems not suitable for generic CAMHS service.



Osborne Court
Osborne Court is part of the provider arm within the Learning Disability integrated service. It operates four distinct services: Children's short break service, Children's emergency and assessment service, Adult's short break service and a Hydrotherapy Pool.

Salaried Dental Services

The Salaried Dental Services provides care from 11 locations within Worcestershire. The service aims to provide high quality dental care to those who have had difficulty accessing NHS dental care in the past. The SDS Provides Out of Hours Care on weekends and bank holidays in Worcester, Kidderminster & Evesham. Dental Care is also provided at HMP Hewell, HMP Long Lartin and HMP Oakwood. Also in conjunction with the Acute Trust care is provided un General Anaesthetic to children and patients with Special Needs. We also have a link into the MDT head and neck oncology team at Worcester Acute.
